Behavioural activation is deceptively simple and remarkably effective: change what you do first, and mood follows. It is one of the best-supported treatments for depression.

Why this happens

Depression establishes a loop: low mood reduces activity, reduced activity removes sources of reward and mastery, and mood drops further. Behavioural activation breaks the loop at the point that is actually controllable. You cannot decide to feel motivated, but you can decide to make one phone call. Large trials have found behavioural activation performs comparably to full cognitive therapy for depression, despite being simpler to deliver — which makes it unusually accessible.

No. Busyness can itself be avoidance. Behavioural activation is targeted, monitored and matched to your values — which activities, in what order, at what size.
What if I genuinely can't get out of bed?
Then we start there, at that scale. Behavioural activation is designed to begin from wherever you actually are, including very low functioning.
How quickly does it work?
Behavioural activation often produces earlier improvement than more cognitively focused approaches, with many people noticing change within three to four weeks.
Is it used alongside other therapy?
Frequently. It is often the opening phase of depression treatment, with cognitive work introduced once energy and activity have improved.
